Please bring the following to each appointment:
* Photo ID
* Insurance Cards
* List of current medications
* New patient forms (if available/applicable)
Patients are requested to always bring their insurance cards, and a list of their current medications to each appointment.
Payments are due at time of service. We accept cash, personal checks, Mastercard and Visa. We do not accept Discover and American Express. If you have a copay, be ready to pay it on the date of your appointment. If you have no insurance, payment will be collected from you at time of service.
